I see a ton of youtube channels with a small banner above the subscribe button in upper right of individual video pages. How can I insert a Subscribe banner above that button?

It's a part of the YouTube Partner program. You get that banner and a removal of the time limit.

I tried signing up for an account a few months back but they turned it down as I "didn't have enough views" for my videos.
